1. Payment Due at Time of Service
Payment is expected in full at the time services are provided, unless prior arrangements have been made.
2. Accepted Payment Methods
We accept:
· Credit/debit cards
· HSA/FSA cards
· ACH bank transfers
· Paypal
· Apple Pay / Google Pay
3. Telehealth Services
Telehealth visits must be paid in advance or at the time of service. You will receive an invoice via email and text prior to your visit with a link to submit payment.
4. House Call Visits
House call visits must be paid in full no later than 24 hours prior to your scheduled appointment (Day prior if appointment is available and scheduled within the 24 hour time period).
If payment is not received by this time, the appointment may be canceled or rescheduled. This ensures we can reserve your time slot and travel resources accordingly.
5. Missed Appointments / Late Cancellations
We kindly ask for at least 24 hours’ notice if you need to cancel or reschedule. Missed appointments or late cancellations may result in a $25 fee, if there is no attempted communication.
6. Direct Primary Care (DPC) Patients
If you are enrolled in our DPC subscription plan, most services are covered by your monthly fee. However, additional services such as labs, procedures, or IV therapy may incur separate charges, which will be discussed beforehand.
