Direct Primary Care (DPC) is the way we deliver that care long-term. It's an option for individuals, families, and small businesses who value access, continuity, and predictable costs - without the complexity of insurance driven medicine. 

Through a monthly membership, patients have an ongoing relationship with their provider, more time during visits, and direct access when questions or concerns arise. Our DPC model allows us to focus on you, simplify care decisions, and remove the barriers that often get in the way of good medicine. 

A Direct Primary Care membership is designed to support your everyday primary care needs with clear expectations and no surprises. 

Depending on your plan, membership may include:

• Unlimited access to your provider through telehealth (video, phone and secure messaging) 
• Same-day or next-day availability during normal business hours
• Annual wellness visits and preventative care
• Chronic condition management and care coordination 
• Select in-home testing, including flu, COVID, strep and ECG
• Transparent pricing with no copays, no hidden fees, no insurance billing

One-time $99 enrollment and onboarding fee

This fee supports onboarding for the lifetime of your membership, care setup and secure access to our practice.  

Enrollment fee waived if joining within 7 days of a visit. 

Virtual Care Membership 

Primary care built around access, continuity and simplicity - delivered virtually 

Our Virtual Care Membership is designed for patients who want and ongoing relationship with their provider and fast reliable access to care, without need for in-person visits. 

This membership includes:

• Unlimited virtual visits (Video, phone, messaging) 
• Same/next-day access
• Chronic & preventative care
• Care coordination
• Transparent pricing

Pricing:

• Adult: $59/month
• Family: $159/month (Up to 4 members, each additional child is $20/month)

This membership does not include in-home visits or in-person procedures.

Direct Care Membership (Central Florida) 

Expanded access with in-person care and home visits when needed

Our Direct Care Membership is designed for patients who want a deeper level of access, including in-person care and the option for care in the comfort of their home. This membership builds on our Virtual Care plan and adds flexibility for patients located in Central Florida. 

This membership includes everything in the Virtual Care Membership, plus:

• One in-home annually 
• Reduced member rates for additional house calls
• In person care when clinically appropriate 
• Priority scheduling 

Pricing:

• Adult: $79/month
• Family: $189/month (Up to 4 members, each additional child is $20/month)

Available to patients located in Central Florida.

Home-Based Care Membership (Central Florida) 

Comprehensive primary care delivered in your home, with the highest level of access

Our Home-Based Care Membership is designed for patients and families who value convenience, proactive care and the ability to receive comprehensive primary care services at home. This membership provides the greatest level of access and continuity for patients located in Central Florida. 

This membership includes everything in the Direct Care Membership, plus:

• Up to four in-home visits per year (approximately quarterly)  
• Up to four in-home diagnostic test per year, including flu, COVID, strep and ECG
• Highest scheduling priority 
• Ongoing care delivered primarily in the comfort of your home

Pricing:

• Adult: $99/month
• Family: $219/month (Up to 4 members, each additional child is $20/month)

Carter Family Practice offers Direct Primary Care Options for small businesses looking for a simpler, more personal approach to employee healthcare - either as a standalone benefit or alongside traditional insurance coverage. 

Our small business plans provide flat, predictable monthly pricing and are designed to reduce healthcare friction for both employers and employees. We work directly with business owners to create a plan that fits their team size, workforce needs, and level of desired access. 

Whether covering a few key employees or an entire team, our goal is to make primary care easy, accessible, and cost-effective. Without long contracts or hidden complexity.

Direct Primary Care doesn't replace how we practice medicine it enhances it for patients who want deeper access and continuity. 

For those who don't choose DPC, the heart of our care remains unchanged.  

A Direct Primary Care membership at Carter Family Practice covers your everyday primary care needs without involving insurance. This means no copays, no deductibles, and no surprise bills for services included in your membership. 

For care outside of primary care, we recommend maintaining a health insurance plan or health-sharing option. Insurance is typically used for services such as: 

• Specialist visits
• Imaging or hospital care
• Emergency services
• Surgeries or major procedures
• Laboratory services not included in your membership

Think of your DPC membership as your foundation of your healthcare, your day-to-day support and point of continuity - and insurance as a safety net for unexpected or higher cost needs.
