Whether you're an individual, a family, or a small business, our Direct Primary Care Plans give you a simple predictable way to experience primary care the way we believe it should be. 

Every Direct Primary Care Plan is designed to give you better access, more time with your provider, and straightforward primary care without surprises. 

Depending on the plan you choose, it may include:

• Unlimited access to your provider through telehealth (video, phone and secure messaging) 
• Same-day or next-day availability during normal business hours
• Annual wellness visits and preventative care
• Chronic condition management and care coordination 
• Select in-home testing, including flu, COVID, strep and ECG
• Transparent pricing with no copays, no hidden fees, no insurance billing

One-Time $99 Enrollment & Onboarding Fee

This one-time fee covers your initial onboarding, care setup, and secure access to our practice. As long as you remain enrolled in your Direct Primary Care Plan, you'll never pay this fee again. 

Enrollment fee waived when you enroll within 7 days of a visit. 

Virtual Care Plan 

Relationship based primary care, wherever you are.

Our Virtual Care Plan is designed for patients who want ongoing access to their primary care provider whiteout the need for routine in person visits.  

Your Plan Includes:

• Unlimited virtual visits (Video, phone, secure messaging) 
• Same day or next day access
• Preventative and Chronic care
• Care coordination
• Transparent, predictable pricing

Monthly Plan:

• Individual: $59/month
• Family: $159/month (Up to 4 members, each additional child is $20/month)

This plan does not include in-home visits or in-person procedures.

Direct Care Plan (Central Florida) 

Everything in our Virtual Care Plan, plus in person care when you need it. 

Designed for patients in Central Florida who want the flexibility of both virtual and in person care, including home visits when appropriate. 

You Plan Includes Everything in the Virtual Care Plan, plus:

• One in-home visit each year 
• Reduced rates for additional house calls
• In person care when clinically appropriate 
• Priority scheduling 

Monthly Plan:

• Individual: $79/month
• Family: $189/month (Up to 4 members, each additional child is $20/month)

Available to patients located in Central Florida.

Home-Based Care Plan (Central Florida) 

Our most comprehensive primary care experience. 

Designed for patients who prefer to receive the majority of their healthcare in the comfort of their home. 

Your Plan Includes Everything in the Direct Care Plan, plus:

• Up to four in-home visits per year 
• Up to four in-home diagnostic test annually
• Highest scheduling priority 
• Ongoing primary care delivered primarily in your home

Monthly Plan:

• Individual: $99/month
• Family: $219/month (Up to 4 members, each additional child is $20/month) 

Available to patients in Central Florida.
