All pricing listed on this page are our self-pay rates, allowing us to provide clear, upfront pricing without hidden fees or surprise bills.
Many services may be eligible for HSA/FSA payment.
For services outside of primary care, such as hospital care, specialist, or advanced imaging, we recommend maintaining separate health insurance coverage.

For new or existing patients experiencing acute symptoms such as illness, infection, or sudden concerns that need timely attention.
Initial visits to establish care, review medical history, and discuss treatment goals. Medication management may be addressed as appropriate.
Ongoing monitoring and management of chronic conditions, including treatment adjustments and medication refills when appropriate.
Brief follow-up calls related to a recent visit are included.
If a concern requires extended discussion, new evaluation, or medical decision-making, a scheduled visit may be recommended.

Comprehensive wellness visits performed in your home, including preventive care, health screenings, and time for meaningful discussion about your overall health. (Includes EKG)

Timely evaluation for acute illnesses or new symptoms that require attention but do not warrant an emergency room visit.
Select diagnostic testing may be performed during house call visits when clinically appropriate, including flu, COVID, strep, and EKG testing.
Testing fees are in addition to the visit cost and are per test.
In-home ear wax removal may be performed during a house call visit when clinically appropriate.

IV fluid hydration provided in the home when clinically appropriate.
Medications may be administered for conditions such as migraines, allergic reactions, or nausea/vomiting, as deemed appropriate by the provider.
Administered via IV or IM injection based on patient needs.
Medication must be pharmacy-labeled with the patient’s name.
Administration is included in the standard visit cost.
CFP provides mobile Department of Transportation (DOT) exams that meet FMCSA requirements — with same-day paperwork, flexible scheduling, and affordable, transparent pricing.
Our DOT Physical Exam includes a full FMCSA-compliant medical evaluation with vision and hearing screening, blood pressure check, and same-day paperwork and certification.
This option includes everything in the standard DOT Physical Exam, along with an on-site 12-panel urine drug screen. All testing and documentation are completed during the same visit, and same-day paperwork is provided.
Companies or organizations needing multiple DOT physicals or drug screenings, CFP offers flexible, on-site group services with convenient scheduling and volume-based pricing. We come directly to your location to complete all exams and drug screens efficiently, with same-day paperwork and medical cards provided for each driver.

In-person physical exams are available for school, sports, and camp requirements. All visits include time to review medical history and ensure forms are completed accurately.
